What is inside

22 pages in five sections: about my dog (profile, baseline, sitter and emergency sheet, vaccines and preventatives), vet visits (visit prep, visit notes, bloodwork and test results, costs and insurance claims), every day and every week (medications and supplements, weekly care log, symptom and event log, behavior and confusion log), every month (weight and body condition, quality-of-life check-in, good days and hard days, care routines), and extras (a page of free resources and four notes pages).

It comes as three files: a one-page Start Here, and the full log in US Letter and in A4. You can print it and write by hand, fill it in on screen with the free Adobe Acrobat Reader and then print or save, or use it fully on screen. Fillable fields work best in Acrobat Reader; other PDF apps and browser previews may not save what you type.

Who it is for

Owners of older dogs who want a tidy record to hand to the vet, and a place to notice trends over weeks instead of trusting memory. If you read our free guides, it is the place to write down the things they tell you to watch. The free Senior Dog Basics pages stay free.

What it is not

It is a record-keeping tool, not veterinary advice. It does not diagnose or treat anything. Write down what your vet told you or prescribed, and call your vet if you are worried about your dog.
